COVID-19 Student Vaccine Information
Sheridan County Public Health will be offering optional COVID-19 vaccinations on our high school campuses to students age 16 and over. Vaccinations are entirely voluntary and will take place on Wednesday, May 5, at the following locations and times:
-SHS Cafeteria – 7:30-8:15 am
-Schiffer Lunchroom – 2:45-3:30 pm
To participate, students must submit this form, completed and signed by a parent or legal guardian, to the school nurse by May 4. There is no cost for the vaccine.
